Abstract:

OBJECTIVE: To investigate the mechanism and target sites of Yishen Qufeng formula (益肾祛风方, YSQF) in alleviating renal injury caused by diabetic kidney disease (DKD), with a focus on the nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3)/ caspase-1/gasdermin D (GSDMD) signaling pathway.

METHODS: A DKD rat model was established using unilateral nephrectomy combined with intraperitoneal injection of streptozotocin. Rats were randomly assigned to four groups: control, model, irbesartan (IRB), and YSQF. The IRB group received oral IRB, and the YSQF group received oral YSQF decoction, while the control and model groups received an equal volume of normal saline. After 12 weeks of intervention, we detected hepatic and renal function-related indicators, such as serum creatinine, alanine aminotransferase (ALT), and 24-h urinary total protein (24-h UTP), as well as serum inflammatory cytokines interleukin (IL)-1β. In addition, histopathological evaluation, including hematoxylin and eosin staining, Masson’s trichrome staining, and terminal deoxynucleotidyl transferase dUTP nick end labeling staining, was performed. The mRNA expression of pyroptosis-related markers in renal tissue was assessed by quantitative real-time reverse transcription polymerase chain reaction, and the corresponding protein expression was assessed by Western blotting.

RESULTS: Compared with the control group, the model group showed significantly increased 24-h UTP and albumin-to-creatinine ratio, as well as elevated serum levels of IL-1β and IL-18. These biochemical alterations were accompanied by more severe renal histopathological damage, increased renal fibrosis, enhanced renal cell apoptosis, and increased NLRP3 immunohistochemical staining intensity. In addition, the mRNA expression levels of NLRP3 and apoptosis-associated speck-like protein containing a caspase recruitment domain (ASC) were significantly upregulated. The protein levels of NLRP3, ASC, cleaved-caspase-1, and the N-terminal fragment of gasdermin D (GSDMD-N) were also significantly increased. In contrast, compared with the model group, the YSQF group demonstrated significant amelioration, including lower 24-h UTP levels. Serum concentrations of IL-1β and IL-18 were also significantly decreased. These changes were accompanied by amelioration of renal histopathological injury, reduced fibrosis and apoptosis, decreased NLRP3 immunohistochemical staining, decreased mRNA expression of NLRP3 and ASC in renal tissue, and reduced protein levels of NLRP3, cleaved-caspase-1, and GSDMD-N.

CONCLUSION: YSQF formula alleviates renal injury in DKD rats by suppressing renal cell pyroptosis viadownregulation of the NLRP3/caspase-1/GSDMD signaling pathway, thereby mitigating inflammatory responses, improving renal pathology, and attenuating renal fibrosis.
