Abstract:

OBJECTIVE: To investigate the potential mechanisms by which Zusanli (ST36) moxibustion alleviates age-related renal lesions viaregulating inflammation.

METHODS: Male mice were divided into three groups: the young group (adult mice aged 3 months), the old group (21-month-old mice maintained until 24 months), and the grain-sized moxibustion (GS-Moxi) group [21-month-old mice treated with grain-sized moxibustion (GS-Moxi) at Zusanli (ST36) for 3 months]. To assess the protective effects of GS-Moxi on age-related renal injury, serum creatinine, blood urea nitrogen levels, and renal tissue were analyzed using periodic acid-Schiff and Masson’s trichrome staining. RNA sequencing (RNA-seq) was conducted for gene set enrichment analysis (GSEA) and kyoto encyclopedia of genes and genomes (KEGG) pathway analysis. Additionally, Western blotting and immunohistochemistry (IHC) were employed to examine the involvement of the phosphatidylinositol 3-kinase/ protein kinase B/nuclear factor-κB (PI3K/AKT/NF-κB) signaling pathway in response to GS-Moxi treatment.

RESULTS: Our findings demonstrated that GS-Moxi significantly improved renal histopathological features and ameliorated age-related renal dysfunction. Additionally, GS-Moxi attenuated the upregulation of fibrosis markers, senescence markers [p16, p21, and senescence-associated β-galactosidase (SA-β-gal)], oxidative stress markers (superoxide dismutase (SOD) and malondialdehyde (MDA)), and inflammatory cytokines [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and interleukin-1β (IL-1β)] in the aging kidneys. Transcriptomic analysis, followed by GSEA and KEGG pathway enrichment, revealed the PI3K/Akt signaling pathway as a critical regulatory pathway. Western blotting and IHC further confirmed that GS-Moxi modulates the PI3K/AKT/NF-κB signaling pathway in the aging kidney.

CONCLUSIONS: Our study confirmed that GS-Moxi at ST36 has a significant therapeutic effect on age-related renal injury by inhibiting the PI3K/AKT/NF-κB signaling pathway, thereby reducing the release of inflammatory factors and mitigating oxidative stress injury.
